WAIVER/INDEMNITY AND POPIA CONSENT FORM
WAIVER AND INDEMNITY
As the parent or legal guardian of a participant in the Youth American Flag Football League ("YAFFL" or "the League"), I understand and acknowledge that participation in flag football, including but not limited to attending practices, matches, and using league facilities and equipment, involves physical activity that may expose the participant to the risk of minor injuries.
Furthermore, I recognize that said activities carry an inherent risk of minor sprains, strains, or other sports-related injuries.
I, as the parent or legal guardian of the participant, represent and warrant that the participant is in good health and does not suffer from any medical condition, which has not otherwise been disclosed by me, that would prevent their voluntary participation and enrolment in any of the above flag football activities.
I further represent and warrant that it is my responsibility to inform the League should the participant's medical condition change to the extent that further participation in any of the abovementioned activities is detrimental to their health or well-being.
I acknowledge and agree that the participant partakes in said activities entirely at their own risk and consequently waive and relinquish all claims I, my successors, or assigns, may have for minor injuries, such as bruises, cuts, or sprains, suffered by the participant as a result of their participation in the League.
I hereby release the League, Club the participant plays for, or any League/Club official or League/Club member from any liability for such minor injuries to the fullest extent permitted by South African law.
I will strictly abide by all applicable YAFFL rules and policies, and otherwise adhere to any instructions issued by YAFFL relating to the participant's participation in the activities.
Further, I shall defend, indemnify, and hold harmless YAFFL and its officials, employees, and agents, the governing provincial and national bodies, for and against any claim of whatever nature instituted against YAFFL or any of its officials, employees, or agents by any person as a result of any incident caused by the participant's willful or negligent conduct or omission, or that of any person for whose actions I am or may be deemed to be responsible.
I shall be liable to YAFFL for any damage or loss caused to YAFFL as a result of the participant's willful or negligent conduct or omission, or that of any person for whose actions I am or may be deemed to be responsible.
By completing the Player Registration Form and ticking the respective box, I, as the parent or legal guardian, agree that the participant will be subject to the Code of Conduct of the League and provincial and national governing bodies.
